If the lower face lies below a line drawn from the top of the ear to the corner of the mouth, then the middle of the face lies above this line and includes the cheek and lower eyelid complex. As we age, the fat pad of the cheek will begin to descend and/or lose volume. This can leave an indentation of the lower orbital area as well as a trough or flatness of the mid portion of the cheek. These undulations in most cases are accentuated by fat pouches from the lower eyelids. As the soft tissues of the cheek sag more, the midface comes to rest on the crease that runs from the side of the nose to the corner of the mouth. This creates a more pronounced situation known as the "Marionette line". All of these changes add to the overall aged appearance of the face and should be addressed with facial rejuvenation surgery.
Whether my recommendation is to fill the cheek with autologous (your own) fat and/or to lift it with one of the newer procedures available, enhancement of the midface adds to the beauty of many of the facial procedures that currently exist today.
